Lenzetto Spray (Prescription Only Medicine)

Clinical Overview

Lenzetto 1.53mg Transdermal Spray is a form of Hormone Replacement Therapy (HRT) containing estradiol, a form of the primary female sex hormone oestrogen. It is indicated for use in postmenopausal women, defined as those who have not had a natural menstrual period for at least 6 months, or in women who have undergone surgical menopause (e.g., bilateral oophorectomy).

The formulation is delivered as a metered-dose transdermal spray, allowing estradiol to be absorbed through the skin into the systemic circulation. HRT should only be initiated for symptoms that adversely affect quality of life.

How Lenzetto Works

Lenzetto delivers estradiol transdermally, where it is absorbed through the skin and enters the bloodstream, providing systemic oestrogen replacement.

Estradiol acts by binding to intracellular oestrogen receptors located in target tissues such as the hypothalamus, bone, and urogenital tract. This receptor activation regulates gene expression and restores oestrogen-dependent physiological processes that decline during menopause.

By replacing endogenous oestrogen levels, estradiol helps stabilise thermoregulatory function in the hypothalamus, which is associated with the reduction of vasomotor symptoms such as hot flushes. It also contributes to the maintenance of bone density and urogenital tissue integrity.

What Lenzetto Is Used For (Licensed Indications)

Licensed indications for Lenzetto include:

  • Relief of postmenopausal symptoms: Including vasomotor symptoms such as hot flushes and night sweats associated with oestrogen deficiency.
  • Use following surgical menopause: In women who have undergone removal of the ovaries, resulting in a sudden reduction in circulating oestrogen levels.

How to Use Lenzetto (Dosage and Administration)

Lenzetto should be used in accordance with the prescribing information provided by a healthcare professional.

  • Starting dose: Typically one spray (1.53mg estradiol) applied once daily.
  • Dose adjustment: The dose may be adjusted based on clinical response, usually up to a maximum of three sprays daily.
  • Application site: The spray should be applied to clean, dry, intact skin on the inner forearm.
  • Administration method: Each spray should be applied to a non-overlapping area of the skin. The solution should be allowed to dry before covering the area with clothing.
  • Contact precautions: Care should be taken to avoid secondary exposure to others, particularly children, through direct skin contact with the application site.

Dosing should be determined by a healthcare professional based on individual clinical need and response. Regular clinical review is recommended to assess the ongoing need for treatment.

Who Should Not Take Lenzetto (Contraindications)

Lenzetto is contraindicated in the following situations:

  • Known, past, or suspected breast cancer.
  • Known or suspected oestrogen-dependent malignancies (e.g., endometrial cancer).
  • Undiagnosed vaginal bleeding.
  • Untreated endometrial hyperplasia.
  • Active or previous venous thromboembolism (e.g., deep vein thrombosis or pulmonary embolism).
  • Known thrombophilic disorders.
  • Active or recent arterial thromboembolic disease (e.g., myocardial infarction, stroke).
  • Acute liver disease or a history of liver disease where liver function tests have not returned to normal.
  • Known hypersensitivity to estradiol or any of the excipients.
  • Porphyria.

Lenzetto Side Effects and Safety Information

Like all medicinal products, Lenzetto can cause adverse reactions, although not all individuals will experience them. Clinical experience in women over 65 years is limited.

Common (≥1/100 to <1/10):

Headache, application site reactions (e.g., irritation), nausea.

Uncommon (≥1/1,000 to <1/100):

Mood changes, dizziness, breast tenderness, abdominal pain.

Frequency Not Known (cannot be estimated from available data):

  • Venous thromboembolism (including deep vein thrombosis and pulmonary embolism).
  • Stroke and myocardial infarction.
  • Endometrial hyperplasia or carcinoma with prolonged use without progestogen.
  • Breast cancer (risk increases with long-term use of combined HRT).

Hormone Replacement Therapy is associated with an increased risk of certain conditions, including venous thromboembolism (VTE), stroke, and hormone-dependent cancers. The risk of VTE is increased, particularly during the first year of use, with studies indicating a 1.3 to 3-fold increase.

In women with an intact uterus, Lenzetto should be used in combination with a progestogen as part of a sequential or continuous combined regimen.

Treatment should be discontinued immediately and medical attention sought if symptoms suggestive of thromboembolism occur, such as sudden chest pain, difficulty breathing, or swelling and pain in the leg.

Lenzetto 1.53mg Transdermal Spray: FAQs

Where should Lenzetto be applied?

Lenzetto is clinically licensed for application to the inner forearm. The skin in this area is typically thin and well-perfused, allowing for consistent systemic absorption of the estradiol. Application should be limited to the recommended site unless otherwise advised by a healthcare professional.

How many sprays of Lenzetto should I use?

The initial starting dose is typically one spray daily. Based on clinical response and the resolution of vasomotor symptoms, a healthcare professional may titrate the dose upwards to a maximum of three sprays daily. Each spray must be applied to a non-overlapping area of the skin to ensure the correct surface area-to-absorption ratio.

How long does it take for Lenzetto to dry?

The spray typically dries within 2 minutes of application. It is clinically recommended to wait at least 2 minutes before covering the application site with clothing, and at least 60 minutes before bathing or showering to ensure optimal absorption into the stratum corneum.

Can I wear sunscreen or moisturiser with Lenzetto?

If you are applying sunscreen or moisturiser to the same area, you must wait at least 60 minutes after the Lenzetto spray has been applied. Applying topical products too soon after the spray may interfere with absorption of the estradiol through the skin.

Is Lenzetto spray the same as Oestrogel?

While both products are transdermal HRT treatments containing estradiol, they differ in delivery mechanism and concentration. Lenzetto is a metered-dose ethanol-based spray delivering 1.53mg of estradiol per actuation. Oestrogel is a hydroalcoholic gel delivered via a pump. Both products deliver estradiol via transdermal absorption, but differ in formulation and dosing, and are not interchangeable without clinical review.

Can I take Lenzetto at night?

Lenzetto can be applied at any time of day, but it should be used at the same time each day to maintain stable systemic oestrogen levels. Many patients prefer morning application to establish a routine, but night-time application is clinically acceptable if it improves adherence or helps manage nocturnal symptoms like night sweats.
